About The Position
Treasure Coast Community Health (TCCH), a long-established and highly respected healthcare provider with locations throughout Indian River County is continuing to expand. We are now seeking a Clinical Care Coordinator to be based at one of our growing patient care sites, to provide clinical, casework and group work services to patients and their families. Will perform a variety of duties to include assessing cases and preparing and implementing treatment plans, counseling and advocacy for patients, coordinating the provision of social services and working collaboratively with medical staff.
