About The Position

The Behavioral Health Therapist (BHT) assists primary care providers in recognizing, treating, and managing mental health, substance use, and psychosocial concerns for patients of Heartland Community Health Center and our community. This position places a emphasis on serving individuals with co-occurring mental health and Substance Use Disorders (SUD).

Requirements

  • Bachelor’s degree and Master’s degree from an accredited School of Social Work (or related discipline)
  • Current Kansas licensure through the Behavioral Sciences Board (LSCSW, LCPC, or LCMFT)
  • A commitment to following all legal and ethical guidelines and standards of the profession and of Heartland Community Health Center
  • Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions
  • The ability to work through brief client contacts as well as make quick and accurate clinical assessments of mental and behavioral conditions
  • Is comfortable with the pace of primary care, working with an interdisciplinary team
  • Good knowledge of psychopharmacology
  • Strong organizational skills
  • Strong oral and written communication skills
  • Strong interpersonal skills in working with diverse publics
  • An energetic and positive approach to the readily evolving changes and challenges of a complex workplace
  • Renew licenses and certifications upon expiration, as required
  • Computer skills to navigate Electronic Health Record software

  • Strong knowledge of integrated treatment models for co-occurring disorders
  • Knowledge of substance use assessment tools (e.g., SBIRT) and DSM-5 diagnostic criteria

  • Knowledge of behavioral medicine and evidence-based mental health treatments
  • Ability to conduct brief interventions and make accurate clinical assessments in a primary care setting
  • Strong organizational, interpersonal, oral, and written communication skills
  • Ability to work effectively with diverse and underserved populations

  • Proficiency in Electronic Health Record (EHR) systems

Responsibilities

  • Conducting assessments, diagnostic and functional evaluations of primary care patients
  • Conduct comprehensive biopsychosocial assessments, including screening and diagnostic evaluations for Substance Use Disorders (SUD) and co-occurring mental health conditions
  • Providing brief, focused intervention for patients in need of mental health services
  • Providing consultation to primary care team providers to enhance skill and effectiveness in treating patients
  • Providing timely feedback regarding care, treatment recommendations and progress of patients via documentation in patients’ charts and through verbal feedback
  • Provide clear written and verbal communications to patient and staff, maintaining a culture of dignity and respect
  • Research and develop programming to meet specific patients’ needs using evidence-based practices
  • Assisting patients with referral to specialty mental health facilities or other community partners and or Assist patients with referrals to specialty SUD treatment providers, detoxification programs, inpatient or outpatient services, and community recovery supports
  • Initiating client follow up, determining if any changes in treatment approach is indicated
  • Developing relapse prevention plans and helping patients maintain stable functioning
  • Assisting in the detection of at-risk patients and in the developing of plans to prevent worsening of their condition
  • Monitoring and coordinating delivery of health services for patients as related to behavioral health care, including linking with other treatment providers within and outside the primary care setting
  • Providing support to patients with or at risk for chronic conditions and helping them make healthier lifestyle choices
  • Assisting clients in complying with medical treatment, offering coping strategies for medication side effects and chronic conditions
  • Regular and predictable attendance is an essential function of the job
  • Completing additional responsibilities as assigned
