About The Position

Stamford Health is seeking a Full-time Clinical Assistant to support our Outpatient Wound Care Center! Hours: 7:30am - 3:30pm M-F Major Responsibilities: Provides direct patient care to a select group of patients and advocates on their behalf. Prepares treatment rooms prior to patient arrival. Assists in the collection and recording of information including, current medications, and allergies, reports results to licensed nurse Checks in patients in the EMR upon arrival, witnesses’ signature, applies patient identifying bracelet after using two forms of identification. Ensures adequate information is available for the RN prior to initial consultation by obtaining medical records timely, obtaining reports and other information as requested. Answers phone calls and manages daily activity of the office under the direction of the licensed nurse. Schedules repeat and new appointments for patients. Documents all activities in EMR according to defined guidelines. Documents pertinent information, observations, and all aspects of patient care clearly, concisely, and in a timely manner Provides care to patients of the department including but not limited to assistance with ambulation and providing comfort items. Bathing, dressing/undressing, bed pans/urinals (toileting), mouth care/grooming and shaving as directed by licensed nurse. Taking vital signs, weights, emptying/maintaining drainage devices, measuring intake and output, as directed by licensed nurse. Assists with prepping patient for procedure. Collecting food trays, providing water pitchers, and assist with feeding as guided by licensed nurse. Assist with transferring patient (bed to chair/chair to bed), ambulation, turning and positioning. Assist in transporting patients. Monitor skin condition and report to licensed nurse. Restock supplies as needed to work area, treatment, and patient rooms Directing visitors as needed. Ability to manage multiple competing tasks, develop effective plan for accomplishing daily tasks and manage priorities and allocation of available resources along with care team. Alternate responsibilities may include cross-training and floating to other identified patient care departments. Requirements

  • Minimum of High School Diploma, GED, or equivalent
