Primary Purpose The Diabetes Patient Care Clinics, funded by Abbott and administered through the Thomas J. Long School of Pharmacy, provide year-round, direct patient care services to the Stockton community. The Clinics focus on screening, prevention and management of diabetes, targeting individuals who are at risk for, or diagnosed with, prediabetes or diabetes. Services include health screenings and testing, patient-centered clinical services, disease-state education, medication management, and strategies to optimize medication access and affordability. Essential Functions Under direction of the Program Director and/or Program Manager of the Diabetes Care Clinics, this position provides advanced administrative and patient-facing support for clinic operations and community outreach activities. This role focuses on patient intake, scheduling, billing, documentation preparation, and customer service, operating under established procedures and guidance from program leadership. The position supports the efficient execution of clinic and outreach activities through accurate data handling and direct patient interaction.
