About The Position

This position is responsible for face-to-face assessments of vulnerable populations in the District of Columbia. Persons being assessed reside in the community or in facilities. The incumbent may also complete review decisions regarding services needed for the person(s) to be successful in the community. This position may also serve as a mentor or preceptor for new staff, assisting in the formal training of assessors and completing quality assurance competency reviews. Additionally, the incumbent may be involved in process improvement initiatives.

Requirements

  • A Bachelor’s in a healthcare or related field is required. A Master’s degree is preferred.
  • A minimum of four years of experience in healthcare. Case management experience preferred.
  • At least one year of experience completing level-of-need assessments is preferred.
  • A Registered Nurse (RN) with a current and unrestricted District of Columbia RN license is required. The License must be current and unrestricted in the contract State.
  • On an active contract, specific required certifications for the assessments and/or quality assurance and training activities being completed are required.
  • Some contracts may require passing SIS Certifications (will be provided by the client).
  • Experience in human services and working with individuals who have disabilities is strongly preferred.
  • Positions working on specific contracts may require a valid driver’s license and reliable transportation.
  • First aid/CPR certification may be required for certain contracts.

Responsibilities

  • Performs face-to-face assessments.
  • Collaborates with health care providers and ancillary service providers to create or validate the appropriate treatment plan.
  • Documents assessment results for the individual member status or negotiates service plan as needed.
  • Completes quality assurance competency reviews and other tasks/projects as assigned, such as new assessor training.
  • Provides ongoing education.
  • May communicate with all healthcare providers to ensure appropriateness of care.
  • Performs miscellaneous duties as assigned.
