About The Position

Acentra Health is seeking a full-time Clinical Assessor to join our growing team in New Hampshire. As a Clinical Assessor at Acentra Health, you will play a key role in serving your community by conducting comprehensive, needs-based eligibility assessments for individuals applying for Medicaid-funded personal care services. These in-person evaluations are conducted in nursing homes, hospitals, and community-based settings throughout Nashua, New Hampshire, and nearby areas, playing a vital role in determining the appropriate level of care and ensuring individuals receive the support they need. As a Clinical Assessor, you will conduct evaluations that guide next level of care and planning, inform placement decisions, and support smooth transitions to the appropriate level of long-term care. Through these essential assessments, you'll play a vital role in promoting patient well-being, enhancing quality of life, and improving overall health outcomes. This hybrid role offers flexibility and purpose, combining remote work from your home office with local travel as needed. Travel expenses including mileage reimbursed at the federal rate and parking are covered by Acentra Health. In addition, Acentra Health provides all necessary technology equipment for your home office and laptop for use in the field.

Requirements

  • Must possess an active, unrestricted RN license with eligibility to practice in the state of New Hampshire.
  • Graduation from an accredited college or university with a degree in a relevant field, along with recent hands-on experience conducting clinical assessments, ensuring accurate evaluations, and providing appropriate next level of care recommendations.
  • Proficiency in medical record abstracting, including a strong understanding of medical record organization, medical terminology, and disease processes.
  • Proficient in using computer-based software and Microsoft Office Suite (Word, Excel, Outlook, Teams, etc).
  • Self-motivated and detail-oriented, with a strong ability to prioritize and execute tasks efficiently while meeting established deadlines.
  • Strong verbal, written, and electronic communication skills, with the ability to convey complex information clearly and interact effectively with individuals and groups

Nice To Haves

  • Familiarity with public mental health and medical programs is preferred.

Responsibilities

  • Conduct thorough clinical assessments of patients, reviewing relevant records and consulting collateral sources to ensure accurate evaluations and well-informed recommendations.
  • Recommend appropriate levels of care and service needs based on a comprehensive evaluation of the patient's physical, behavioral, and psychological functioning.
  • Complete assessments promptly and within the scope of practice, meeting all contractual and regulatory requirements.
  • Demonstrate strong typing and computer skills, with proficiency in Microsoft Word, Outlook, and other relevant software.
  • Adhere to program workflows and ensure compliance with established processes and standards.
  • Perform person-centered assessments, providing meaningful and well-considered recommendations for levels of care and support services.
  • Leverage clinical expertise and knowledge of psychiatric disorders as outlined in the latest Diagnostic and Statistical Manual (DSM) applicable in New Hampshire.
  • Apply knowledge of mental health standards of practice, as well as intellectual and developmental disabilities, geriatrics, and conditions commonly requiring long-term care services and supports.
  • Maintain a high level of attention to detail, prioritize tasks effectively, and execute responsibilities in a timely and efficient manner.
  • Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities.
  • Collaborate with healthcare professionals, patients, families, and community support systems to ensure optimal care and outcomes.
  • Ensure compliance with necessary workflows to maintain quality and efficiency.
  • Demonstrates flexibility in adapting to changing priorities, with strong organizational skills and the ability to efficiently prioritize and execute tasks with attention to detail.
