About The Position

The Clinical Assessment Manager at CenterLight Health System is responsible for overseeing the clinical assessment process, ensuring compliance with Department of Health policies and regulations related to the Uniform Assessment System of New York (UAS-NY). This role involves conducting assessments, managing documentation, and collaborating with various teams to facilitate the enrollment and care delivery for participants in the PACE program. The manager will also engage in quality improvement activities and provide clinical insights to enhance participant outcomes.

Requirements

  • Registered Nurse NY State License, BS in Nursing or other health-related area preferred.
  • 3-5 years of experience in MLTC, LTHHCP, MAP, PACE, or other managed care programs.
  • 5+ years of experience in care/case management, disease management, or related fields.
  • 1 year of experience working with a frail or elderly population, or appropriate training upon hiring.
  • Proficiency in utilizing and analyzing UAS-NY and Lenavi.
  • Experience with Customer Relationship Management platforms and clinical management systems.

  • Bilingual skills are strongly preferred.

Responsibilities

  • Conduct in-home assessments for PACE participants using UAS-NY to determine eligibility for Community Based Long Term Care Services.
  • Evaluate and review previous assessments to ensure accurate documentation in the electronic medical record (EMR).
  • Promote and educate potential referral sources about the PACE program to encourage participation.
  • Educate potential/new PACE participants on plan benefits and services.
  • Escalate questions or concerns during assessments to the quality review team.
  • Complete and/or reassess previous PCAT Tasking Tool to determine appropriate home assistance needs.
  • Develop proposed service plans during home visits and document all interactions in the clinical management system.
  • Work with the quality review team to ensure documentation accuracy in the Health Commerce System and EMR.
  • Maintain knowledge of CMS/PACE guidelines and ensure timely completion of UAS-NY assessments.
  • Provide clinical information to the Care Delivery team for smooth transitions of new participants.
  • Engage in continuous quality improvement activities to enhance staff performance and participant outcomes.
