About The Position

Join the Clinical Ambulatory Pharmacy (MTM) team in making rural life healthier! This position will be split between Albany and St. Joseph Family Medicine clinics where you will help establish a new MTM practice. Seeking individuals with MTM experience. The Clinical Ambulatory Pharmacist will conduct comprehensive medication management services for CentraCare clinic, will adhere to Ambulatory Pharmacist Scope of Practice standards while ensuring that pharmacy laws and other regulatory requirements are met, and will precept and provide mentorship for the ambulatory pharmacy resident(s) and student(s).

Requirements

• PharmD degree and/or BS Pharm degree required.
• 1 year PGY-1 Pharmacy Practice Residency preferred.
• Current licensure by the Minnesota State Board of Pharmacy or eligibility for licensure required.
• Eligible for Pharmacy Specialty Board Certification preferred.
• Basic Life Support (BLS) Certification within 90 days required.
• Previous ambulatory pharmacy (MTM) experience preferred.

Responsibilities

• Ensure compliance with pharmacy laws and regulatory requirements.
