The Patient Navigator focuses on the patient's needs and performs a variety of health education, clerical and medical support services within the clinic setting and in the community. A primary function of the navigator is to establish and maintain communication between patients, their families, physicians, and the health care system to help facilitate care coordination that patients require.

Requirements

  • Five (5) years previous experience in a provider clinic setting or additional education or equivalent (i.e., Bachelor's) with progressive experience required.
  • Must obtain a license or become certified/registered to practice as a CNA/MA/LPN in the State of Colorado within 18 months from the date of hire.
  • Ability to establish cooperative working relationships with leaders, physicians, peers, and the public.
  • Demonstrated ability to maintain composure when confronted by difficult situations and to respond professionally.
  • Demonstrated ability to work as part of a team while functioning independently.
  • Experience with various computer software and hardware including word-processing, knowledge of Microsoft Word, Excel, Outlook, and the internet.
  • Possesses strong customer service skills to promote pleasant and effective interactions.
  • Knowledge of medical terminology, and EMR required.
  • Fluency in English (speaks, read and write) is required; Fluency in Spanish (speaks, translate, read and write) is preferred.
  • BLS is required.

Responsibilities

  • Responsible for new patient intake appointments, patient care plans, medication prior authorizations and appeals.
  • TCM phone calls-submit referral to needed specialties.
  • Responsible for patient outreach for preventive or chronic care services.
  • Ensure patients get to their appointments by reminding them of the day and time 2-3 days before appointment.
  • Assist with any visit coordination that supports improvement in hypertension care and diabetes care.
  • Liaison for the patients and physicians in the practice by assisting patients in accessing service within the system.
  • Facilitate communication with referring physicians, referring patients to appropriate community resources.
  • Provide follow-up clinical orders and scheduling.
  • Complete background checking on all controlled substances, PDMP verification.
  • Manage all home health patients to include completion of billing forms required for certification and re-certification.
  • Clerical duties include composing and typing routine correspondence, copying and filing correspondence, routing incoming mail, ordering and maintaining supplies.
  • Interview patients, measure vital signs, and record information on patient's charts.
  • Room patients, obtain clinical information such as medication list, patient concerns, questions and height and weight.
  • Prepare patients for and assist with examinations.
  • Maintain records of vital statistics and other pertinent data of patient.
  • Act as a scribe for the provider when requested.
  • Perform clinical documentation in EHR system.
  • Make phone calls to patients regarding test results or medication changes.
  • Administer injections and medications, dress wounds and incisions, perform blood draws.
  • Anticipate patient needs and respond to patient requests promptly.
  • Assist with scheduling of patient appointments, answering incoming calls, typing and filing correspondence.
  • Assist with special assignments as necessary.
  • Responsible for extracting old records from EHR systems and adding items to Athena Health.
  • Responsible for pulling health information from QHN and entering the data.
