Requirements

  • Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program OR Bachelor of Science (B.S.) AND a minimum of 2 years relevant experience.
  • Commitment to excellence, accuracy, attention to detail and teamwork required.
  • Ability to communicate effectively, work collaboratively with others, organizes time well, practice with an agile mindset, solve problems, and work independently with minimal supervision required.
  • Current pharmacist license by the Board of Pharmacy in the state(s) of the job's practice site, or eligible to obtain within a time frame compliant with the site's applicable employee licensure policy (60-days after start date). May not independently fulfill responsibilities reserved only to a pharmacist until license is obtained.

  • Other advanced degrees (e.g., Master's, Ph.D.) and applicable Board of Pharmacy Specialties (BPS) or other certification applicable to the position as determined by the supervisor.
  • Completion of pharmacy residency and specialization (e.g., PGY-2 and respective BPS certification), if applicable.
  • Evidence of ongoing scholarly pursuits including research/quality improvement, publications, or scientific presentations.
  • Relevant applicable pharmacy experience working in a progressive and collaborative practice environment.
  • Experience and/or interest in teaching and/or precepting.

Responsibilities

  • Provides medication management services in coordination with other health care professionals to deliver safe, appropriate, and cost-effective medication therapy to patients.
  • Responsibilities may include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services.
  • Participates in collaborative practice and/or protocol-driven practice as appropriate.
  • Pharmacists are active members in quality improvement, research activities, and education as described in the job specific competencies.
  • Provides high-quality pharmaceutical care through the accurate interpretation and dispensing of medication orders in the outpatient pharmacy
