Under the supervision of the Clinic Manager, performs assigned duties relating to the implementation
and evaluation of community health worker services.
Interview patients, measure vital signs, including weight, blood pressure, pulse, temperature, and document all information in patient’s chart.
Provide instructions to patient as directed by physician.
Provide excellent Customer Service to all clients / participants
Triage patients and assist physician in exam rooms.
Conduct and secure prior authorizations for patient’s medication and treatment.
Provide adult immunizations using the most recent Center for Disease Control and Prevention guidelines.
Obtain specimens for hepatitis B and C testing.
Provide STD testing with provision of results to the client under the direction of NJ’s STD program.
Treat individuals with positive results as appropriate, or refer to a health care provider and report all reportable positive results to the local health department.
Provide point of care pregnancy testing with provision of results to the client.
Refer 100% of pregnant women not already linked to obstetrical care to care.
Provide education on basic wound management services, overdose prevention and safer injection techniques.
Maintain an appropriate medical record for all patients as required by the New Jersey Board of Nursing and the Board of Medical Examiners.
Ensure all related reports, laboratory tests and information is filed and is available in patients’ medical records prior to their appointment.
Keep exam rooms stocked with adequate medical supplies, maintain instruments, and prepare sterilizations as required.
Maintain all logs and required checks (i.e. refrigerator temperatures, emergency medications, expired medications, oxygen, etc.)
Arrange for medical oversight by a physician as required by the New Jersey Board of Nursing and the Board of Medical Examiners.
Arrange for the provision of standing orders for all immunizations and laboratory testing by a physician with an active license from the New Jersey Board of Medical Examiners as well as appropriate malpractice coverage.
Establish, maintain, and update client case records using approved agency forms and procedures
Ensure confidentiality of all records
Prepare monthly reports in accordance with agency policies and procedures
Maintain and update client information in EMR
Attend and participate in staff meetings and supervisory conferences
Maintain working relationships with other community providers
Observe established administrative procedures
Provide written reports and maintain statistics as required
Represent the agency at community events, meetings, and conferences
Adhere to Hyacinth policies and procedures including customers confidentiality provisions
Attend and participate in staff training opportunities

A minimum of four years’ experience in the field of HIV / AIDS or social service support
