About The Position

We are looking for a Licensed Practical Nurse (LPN) to join our dynamic primary care clinic team and support our mission of providing unique, compassionate care for seniors. The Program of All-Inclusive Care for the Elderly (PACE) offers older adults and their caregivers a comprehensive alternative to nursing home care, with access to medical services, social support, and care coordination while allowing participants to enjoy independent living in their own homes and communities. The Licensed Practical Nurse (LPN) provides essential clinical support to our busy primary care clinic. In this full-time role, you will administer medications, monitor immunizations, maintain accurate electronic health records (EHR), assist the medication room nurse, and support staff CPR recertification. You will also participate in the nursing on-call rotation to provide after-hours clinical support. The ideal candidate is detail-oriented, organized, able to multitask in a fast-paced environment, and demonstrates compassion and patience while working with seniors, including those with dementia. The clinic LPN will share on call responsibilities with the Case Manager RN’s, which consists of handling issues which arise after-hours and on weekends. On Call is one week in duration, and frequency depends on the number of staff; currently on call will be 9 with this position filled. The physicians/NPs are also on an on-call rotation with the nurses, so you have coverage as needed.

Requirements

  • Holds and maintains a current LPN Nurse license with the State of Oklahoma.
  • Holds and maintains a valid Oklahoma driver’s license, proof of vehicle insurance, clean driving record, and use of personal vehicle.
  • Minimum of one (1) year of documented experience providing care to a frail or elderly population.
  • Experience in the provision of nursing care, including clinical skills, health education, and current nursing treatments and protocols.
  • Working knowledge of the physical, mental, emotional, spiritual, and social needs of frail elderly participants and their families.
  • Ability to work effectively with ethnically and culturally diverse populations.
  • Ability to collaborate effectively with interdisciplinary care teams.
  • Excellent interpersonal, written, and verbal communication skills.
  • Strong typing skills with accuracy in spelling, punctuation, and grammar.
  • Demonstrated discretion, judgment, organizational skills, initiative, and ability to work independently.
  • Ability to prioritize tasks effectively while functioning as a collaborative team member.
  • Proficient in Microsoft Word, Excel, PowerPoint, and Outlook.
  • Consistently professional, positive, and approachable demeanor.

Responsibilities

  • Prepare and administer medications to participants as ordered by the physician or provider in a safe, accurate, and timely manner.
  • Demonstrate knowledge of medications, including actions, indications, side effects, and provide participant/caregiver education as needed.
  • Document medications administered, dispensed, or received in the EHR, including those received from the pharmacy.
  • Report and follow up on medication errors, adverse reactions, or contraindications in accordance with LIFE PACE policies.
  • Monitor participant immunization status and provide accurate data for required quarterly reporting.
  • Coordinate and facilitate annual influenza vaccinations and other recommended immunizations for PACE participants.
  • Maintain vaccination consents and current Vaccine Information Statements (VIS) for administered vaccines.
  • Support staff CPR skills testing and recertification in coordination with the medication room nurse.
  • Assist clinic staff with EHR service tracking and task completion.
  • Complete and maintain required clinical logs for quality assurance and reference.
  • Adhere to HIPAA and HITECH regulations and complete mandatory annual training.
  • Attend staff meetings, in-services, and special projects as assigned.
  • Participate in the nursing on-call rotation as scheduled.
  • Perform other related tasks as assigned by the supervisor.
