The Licensed Clinical Social Worker (LCSW) in Oncology provides direct assessment, diagnosis, formulation, treatment planning, treatment intervention, and education to patients and/or their family members within a clinic setting. This role is based on the Patient Centered Medical Home (PCMH) model, which supports the elimination of barriers to access, including psychosocial problems. The Hematology & Oncology teams at Samaritan Health Services specialize in diagnosing and treating adult cancer patients using chemotherapy, hormonal therapy, and biological therapy, as well as treating a range of hematologic conditions. The LCSW will educate patients about their illness, treatment options, and provide supportive care during treatment and into survivorship.
