About The Position

EmpowerMe Wellness is seeking a licensed therapist to join their innovative care coordination program as a Clinic Director. This role offers the opportunity to practice at the top of your license while expanding into care coordination duties, providing standardized hours, and fostering career growth. The position involves working with residents in senior living facilities across various levels of care, including Independent Living, Assisted Living, Memory Care, and Skilled Nursing, to enhance their independence, functional ability, and overall well-being. EmpowerMe Wellness is dedicated to improving the lives of seniors by providing fully integrated healthcare, including on-site care coordination, therapy, and pharmacy services, through a team of over 3,500 clinicians and professionals nationwide.

Requirements

  • Degree from an accredited OT, PT, COTA, PTA, or SLP program
  • Active ASHA Certification Required for SLP candidates
  • Current state licensure in OT, PT, COTA, PTA or SLP
  • Excellent verbal, written and interpersonal communication skills
  • A results-oriented mindset with strong critical thinking and problem-solving skills
  • A tech-savvy skillset with the ability to learn quickly
  • A sense of professional curiosity, with a desire to learn new things, and to find/recommend solutions to problems

Nice To Haves

  • A minimum of one year management experience preferred

Responsibilities

  • Oversight of patient care in an assigned community or communities in accordance with Federal, State, and Local regulations.
  • Providing services (Therapy and/or Wellness) and operational support to the rehabilitation service teams for implementation of company-wide policies, clinical programs, and quality initiatives to optimize each patient's functional well-being and satisfaction.
  • Advising treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices, and ethical standards (as Clinic Director).
  • Providing a full range of therapy services including assessment, treatment planning, and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices, and ethical standards (as a therapist).
  • Contributing to program development, quality improvement, and problem-solving in accordance with company-wide policies and clinical initiatives, to optimize each patient’s functional well-being and satisfaction.
