About The Position

The Clinic Care Coordinator (CCC) plays a key role in the daily operations of the School-Based Health Center (SBHC), ensuring a smooth and welcoming experience for patients and supporting both clinical and non-clinical staff with essential administrative functions. Primary responsibilities include patient registration, check-in, scheduling, and managing key administrative tasks such as verifying eligibility, handling medical records, and processing referrals. The CCC also collaborates closely with school staff to develop and maintain systems for student referrals, parent consent, and managing student flow to and from class for appointments. Additional duties include assisting with data tracking and reporting, ordering and maintaining inventory of office and medical supplies (including vaccines), and helping to promote awareness of the SBHC services to students, families, and the broader school community. The CCC may also assist families with insurance eligibility and applications, helping reduce barriers to accessing care.

Requirements

  • High School Diploma or GED required, BA preferred.
  • Four (4) years' experience in a clinical setting, with one (1) year in a lead role.
  • Ability to speak and write fluently in one of the following languages, Spanish (highly preferred), Vietnamese, or an East African Language is valued.

Nice To Haves

  • Experience serving patients from a wide range of backgrounds, including those who speak limited or no English is preferred.
  • Experience working in a medical or dental clinic preferred.
