About The Position

MANA Family Medicine HarBer is seeking a Clinic Care Coordinator (LPN or RN) to lead and support initiatives in Value-Based Care and Population Health. You'll play a pivotal role in improving health outcomes by coordinating patient care, closing quality gaps, and helping at-risk patients navigate services. This role supports MANA's mission to provide exceptional, coordinated care to the people of Northwest Arkansas. This is a full time position Monday-Thursday 8-5 and Friday 8-12. The ideal candidate will possess strong communication and interpersonal skills, be highly organized and self motivated. Must be able to work as a team and individually, as well as possess strategic and critical thinking skills.

Requirements

  • Graduation certificate from accredited LPN / RN School of Nursing
  • 1-2 years related experience and/or training
  • LPN or RN active license

Nice To Haves

  • Prior Family Medicine experience preferred, but not required

Responsibilities

  • Drive performance on Value-Based Care programs and population health initiatives.
  • Review and act on quality metrics and performance data for attributed patients.
  • Conduct patient outreach, care gap closures, and referrals while maintaining HIPAA compliance.
  • Educate providers and staff on quality measures, coding/documentation best practices.
  • Collaborate across internal and external care teams and payer systems.
  • Submit data through EMR systems and payer portals to close gaps in care.
  • Support patients' care plans, resource access, and social determinant assessments.
  • Promote patient self-management and empower patient/families to achieve maximum levels of wellness and independence.
  • Responsible for working with patient and patient's care team to coordinate change readiness, needs assessment and develop an individualized treatment care plan.
  • Collaborate with the patients, physician, and other care teams' members in assessing the patient's progress toward individual health care goals.
  • Assess barriers when patient has not met treatment/goals, is not following treatment plan of care, or has not kept important appointments.
