About The Position

The Clinical Pharmacy Coordinator at Clark Regional Medical Center is responsible for overseeing pharmacy operations, ensuring medication safety, and providing comprehensive pharmacotherapy for inpatients. This role involves managing departmental functions, developing drug therapy protocols, and collaborating with healthcare professionals to optimize patient care. The position emphasizes personalized patient care and supports professional growth within a community-focused healthcare environment.

Requirements

  • Graduate from an ACPE accredited school of Pharmacy
  • Active Pharmacist license in the state of KY
  • At least 3 years of hospital pharmacy experience and/or completion of PGYI/PGY2 residency
  • Doctor of Pharmacy (preferred)
  • Basic Life Support (BLS) certification within 10 days of hire
  • Advanced Cardiovascular Life Support (ACLS) certification within 30 days of hire

Responsibilities

  • Assist in departmental management, including operations, patient care delivery, clinical service determination, and regulatory compliance.
  • Create and manage evidence-based, rational, cost-effective drug therapy protocols and pharmacy policies.
  • Provide comprehensive, patient-centered pharmacotherapy for inpatients, making specific drug therapy recommendations.
  • Conduct proactive reviews of patient profiles and participate in patient care rounds.
  • Contact and confer with providers to optimize medication therapy and complete pharmacotherapeutic consults.
  • Participate in the medication reconciliation process at care transitions and promote economically conscientious use of medications.
  • Document all pharmacy interventions and outcomes.
  • Provide pharmacy operation staffing support and review medication orders for accuracy and completeness.
  • Identify, prevent, and resolve potential or actual drug-related problems.
  • Supervise technical staff to ensure quality and accuracy of dispensed products.
  • Manage patients in an outpatient anticoagulation clinic, including operation of Point of Care testing and patient education.
  • Act as an information resource regarding medication therapy and drug-related information.
