About The Position

The Client Coordinator works closely with clinicians, other MHTs, and administration staff to aide getting the clients checked in and checked out of the detox facility. This position focuses on the admissions process and completing the discharge process. The Client Coordinator will also provide case management services through the development of discharge planning and all the components that allow for smooth coordination of care. Who is Axiom Care? Founded in 2012, Axiom Care is a Phoenix-based behavioral health organization that aims to ensure every Arizonan has access to compassionate, effective substance use treatment. With facilities in Maricopa and Pinal Counties, we offer comprehensive services including 24/7 observation, inpatient detoxification, residential treatment, outpatient treatment, and recovery housing with a focus on underserved populations. Beyond our full-continuum community programs, we proudly operate specialty programs for the justice-involved (in partnership with ADCRR), for members of the Navajo Nation (in partnership with the Navajo Department of Health), and for Veterans (co-located on the Victory Place campus). Requirements

  • Must be proficient in computer applications and electronic medical record systems.
  • Proficient with Microsoft Office Suite or related software.
  • Current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR).
  • Ability to communicate effectively with clients, their loved ones and professionals.
  • Ability to make sound decisions in emergency situations.
  • Strong assessment skills.
  • Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Working knowledge of twelve step programs of recovery.

  • High level of patience
  • Active CPR and First Aid certification required (may be provided by Axiom).
  • Active Fingerprint Clearance Card
  • Negative TB Test result within past 12 months

Nice To Haves

  • Detox: 1 year (Preferred)
  • Strong computer skills required, experience with electronic medical records preferred.
  • Strong assessment skills.
  • Minimal supervision regarding use of time.
  • Ability to communicate effectively with clients, their loved ones and professionals.

Responsibilities

  • Actively participate as a positive member of an interdisciplinary treatment team.
  • Complete accurate documentation same day as date of service.
  • Maintain good standing with outside (referral) agencies to be able to provide placement options to our clients
  • Stay current with updated research and best practices.
  • Be aware of community referral sources and refer when necessary.
  • Secure client information per HIPAA standards.
  • Provides and attends in-service training as needed.
  • Complete case management functions focused on a housing first and high needs case management model.
  • Complete AHCCCS application and employment specialty services paperwork.
  • Develop client-driven discharge plans and continually monitor and record discharge plan progress.
  • Collaborate with peer employees, other housing staff and management to ensure fidelity to program structure and guidelines.
  • Assist clinical, housing, and support staff in coordinating care with external sources.
  • Works as part of highly communicative treatment team and supports staff in promoting treatment goals and values of the Axiom Care program.
  • Complete documentation in the electronic medical record of activities, interventions, client contact, client staffing and case management treatment planning.
  • Attend frequent site meetings with peers, clients (house meetings), and ongoing staff trainings.
  • Transport clients to appointments as needed.
  • Obtain Vital signs, UAs, and BAL.
  • Create ROIs for clients’ HIPPA protection.
  • Communicate with the RNs and assist with any patient needs.
  • As part of Axiom Care’s commitment to Culturally and Linguistically Appropriate Services (CLAS), this position supports efforts to provide inclusive and accessible translation services for clients. Responsibilities may include participating in CLAS-related training, supporting language access initiatives, and promoting cultural sensitivity in day-to-day operations.
