The Client Coordinator will provide program planning and support for clients within the Supported Living program in Bloomington, IN. This role involves working with an interdisciplinary team to assess, develop, and implement Individual Support Plans while ensuring compliance with state regulations and monitoring client health and safety.

  • Provide program planning and support to clients in the Supported Living program.
  • Work with interdisciplinary team members to assess and develop Individual Support Plans.
  • Monitor client health and safety.
  • Manage and maintain client benefits including Social Security, Medicaid, and Medicare.
  • Oversee the overall operation of assigned locations and ensure compliance with state regulations.
  • Visit homes regularly and lead a team of Site Supervisors and Direct Support Professionals (DSPs).
  • Bachelor's degree in a related field, or a High School Diploma/G.E.D. with two years of experience in services to persons with developmental disabilities.
  • Management and supervisory experience required.
  • Driver's License (Required).
  • Experience supporting people with disabilities for at least 2 years.
