We are looking for a compassionate and organized Client Care Manager with the dedication and professionalism it takes to work for SYNERGY HomeCare of Apollo Beach. SYNERGY HomeCare offers private duty, non-medical home care services to elderly and disabled clients in the comfort of their own residence. We work as a team to achieve high standard in-home care.

Requirements

  • Home care or Health care experience preferred
  • Strong phone skills
  • Ability to multi-task and work well under time pressure
  • Positive, friendly attitude and ability to work well in a close, team environment

Responsibilities

  • Conduct New Client Care Assessment and Care Plan Development
  • Respond to questions regarding client and caregivers' schedules
  • Notifies clients with basic information on caregivers, service start date and times caregivers will be providing care
  • Develops and documents Client Care Plans
  • Familiarize caregiver with client information, direction, time and basic introduction of clients need and diagnoses
  • Completes Assessment document
  • Document skills and attributes of caregivers to ensure strong matching to clients
  • Update the office team on scheduling issues and HR recruitment needs
  • Maintain computer schedules and ensure timely data entry for clients and caregivers
  • Maintain confidentiality of all information pertaining to employees and clients
  • Perform other functions as deemed appropriate by the management team
