About The Position

We are seeking a dedicated and dynamic Client Care Coordinator to lead daily operations within our FlexHome program at The Palms. This role is ideal for someone who thrives in a fast-paced environment, enjoys building relationships, and is passionate about delivering exceptional care while growing a program.

Requirements

Strong organizational and scheduling experience
A relationship-driven professional with excellent communication skills
Ability to lead, train, and motivate caregivers

Passion for improving the lives of seniors and supporting their independence

Experience in home care, senior living, or healthcare preferred

Responsibilities

Manage daily scheduling to ensure seamless care delivery for all clients
Serve as the primary point of contact for clients, families, and community partners
Provide hands-on care support when needed to maintain service excellence
Oversee program operations and ensure high-quality standards are consistently met
Train and support caregivers, ensuring proper skill alignment and client matching
Actively engage with residents and prospects to grow program census
Monitor performance metrics and drive results aligned with key performance indicators
