About The Position
We’re looking for a compassionate, dependable Client Care Coordinator to join our growing home care team! This unique role is perfect for someone who enjoys variety and is eager to support both clients and caregivers. Ideal for someone who thrives in a dynamic, people-centered environment. The Client Care Coordinator will assist in ensuring our clients are receiving the best possible care. They will assist in development of a Care Plan as part of the initial client intake session and keep it up-to-date as a result of subsequent client visits. They will participate in staffing decisions with the goal of optimizing both caregiver/client fit and completion of the Care Plan. They will also be available to cover shifts as needed for emergency situations and call-offs.
