Our multi-disciplined treatment team works with individuals ages 18+ who experience severe and persistent mental illness, co-occurring developmental disabilities and substance use disorders. The Client Benefits Specialist helps clients apply and/or reapply for state funded benefits so that individuals are able to access health services to improve quality of life. This is a full time, 40 hour per week position.

Requirements

Bachelor’s degree in counseling profession preferred (social work, mental health counseling, human services) however associate's degree with experience, knowledge, and skills to successfully perform the duties of the position will be considered.
Ability to interpret Federal, State, and local laws, regulations, and administrative code about public benefits.
Ability to exercise sound clinical judgment in routine, urgent, and emergent crisis situations.
Proficient computer skills, specifically with all Microsoft office applications.
Ability to document effectively and efficiently in course of work.
Ability to navigate an electronic health record.
Ability and desire to work as part of a team while also able to be self-directed and motivated with minimal supervision.
Ability to acquire and effectively practice new skills as needed according to evolving agency needs/services.
Effective communication skills, organizational skills, and ability to multi-task.
Reliable internet access, transportation, a clean driving record, and proof of auto insurance coverage is required.

Responsibilities

Provide eligible clients with benefits support services to successfully follow through with the benefits application and redetermination process.
Provide education and support with applying to benefit resources such as but not limited to Aid to the Permanent and Totally Disabled (APTD), Women, Infant and Children’s (WIC) Nutrition Program, Social Security, and Medicaid.
Help clients set up NH Easy accounts to manage their Medicaid benefits.
Assist clients and families in gathering proofs, completing necessary paperwork, and following through with applications or renewals for NH Medicaid.
Provide follow-along support to assist clients with managing their benefits, report income, and access available work incentives.
Work collaboratively with members of the treatment team, Intake Clinicians, Social Security Administration, Department of Health and Human Services (DHHS), clients, natural supports, and other community partners to ensure a wrap-around approach is provided for continuity of client care.
Engage in appropriate consultation meetings and committees, both internal and external, as determined by supervisor.
Function professionally and in a manner that protects the integrity, confidentiality and rights of all clients.
Execute all required services and timely documentation in accordance with agency policies and federal/state standards.
Maintain professional memberships, licenses, accreditation, certifications, etc.
Represent CLM in a professional and appropriate manner in all settings.
Perform all other duties as assigned by supervisor.
