Community Health and Behavioral Services (CHBS) is an OMH Article 31 out-patient Mental Health Clinic serving children, families and adults in the community. We are looking to grow and expand services in the community. To keep up with the demands for our services, our team is looking for professionals to work as a part of an interdisciplinary team to meet the needs of our clients in a high-paced clinic setting. The Client Access Representative (CAR) works within the Community Health and Behavioral Services (CHBS) program to support clients in accessing CHBS services. The CAR is the first staff that new, established and returning clients interact with by the phone or in person. The CAR facilitates rapid access to services by reducing barriers to scheduling. The CAR is a key member of the CHBS interdisciplinary team and facilitates communication between clients and their care team, to include nurses, medical and clinical providers. The CAR processes referrals, intakes, client registration, answers phones, schedules, takes messages and facilitates general communication. The CAR coordinates follow up access to care with community referral sources, including hospitals and treatment centers.
