About The Position

Overview Department: Inpatient Social Work Schedule: Full time | Monday-Friday | 8:00-4:30pm CST Facility: Prime Healthcare St. Mary of Nazareth Hospital Location: 2233 W Division St, Chicago, Illinois 60622. Free parking Responsibilities Responsible for the coordination of the various activities of the Case Management Department including utilization management, social services and performance improvement. Assists with development of discharge plans. Assists with case management referrals and authorizations. Maintains and coordinates Medi-Cal (TARS) for presentation to on-site reviewer and files them accordingly or as per facility’s procedure. Assists with Discharge Planning as assigned by interviewing patients/families and gathering data to develop a safe Discharge Plan. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Knowledge of Discharge Planning/ Utilization Management / Case Management terminology and functions, in both managed care and non-managed care environments. 2. Experience in basic to intermediate computer skills/knowledge 3. Current BCLS (AHA) certificate required upon hire and maintain current. Facility Specific Requirements (facility may require items listed below): 1. RN license upon hire preferred 2. LVN license upon hire preferred with 5 years of experience.
