About The Position

The Clerical Associate III (CA III) position is part of the Critical Time Intervention (CTI) team, which supports patients with complex needs in achieving full and healthy lives. The role focuses on promoting community integration, enhancing self-efficacy, and building a robust network of formal and informal supports. The CA III facilitates and implements plans to help consumers reintegrate into the community post-hospital discharge, providing flexible patient-centered support in both clinical settings and in the field as needed.

Requirements

  • A four-year High School Diploma or its educational equivalent.
  • One year of satisfactory clerical experience.
  • Keyboard familiarity with the ability to type at a minimum of 100 keystrokes (20 words) per minute.

Responsibilities

  • Facilitates and implements plans to help consumers reintegrate into the community post-hospital discharge.
  • Supports the CTI team which provides direct services to assist clients emerging from a behavioral health crisis.
  • Specializes in back-end clerical operations, including creation and maintenance of Epic reports and providers' templates.
  • Schedules follow-up appointments and answers client phone calls, directing them to the proper sources.
  • Monitors messages via Epic and takes appropriate action.
  • Pre-registers patients scheduled for upcoming appointments, completing/updates demographics and visit authorizations ensuring accuracy.
  • Works closely with Critical Time Intervention team lead and other departments, including Finance to improve collections.
  • Collaborates with clinical staff to ensure patients receive the best possible care.
  • Participates in meetings and huddles when necessary.
  • Interacts and communicates with patients with Behavioral Health needs.
  • Practices contingency plans during emergencies such as IT and power outages.
  • Monitors compliance as per OMH, JC, and CMS standards.
  • Provides HIPAA compliant patient appointment information with agencies and other sites as required.
  • Monitors equipment and clinic environment, submitting work orders as needed.
  • Assists with Performance Improvement & Quality Assurance reports and data collection.
  • Calls patients to remind them of upcoming appointments.
  • Responsible for Front Desk assignments when needed, including greeting, registration, scheduling, check-in/check-out.
  • Monitors show rates closely and shares with providers.
  • Requisitions in-house supplies.
  • May assist in other units within Behavioral Health as needed.
  • Attends regular team meetings to discuss patient progress, update care plans, and communicate with patients' healthcare providers.
