The role involves providing dementia care, including adapted activities of daily living (ADLs), meaningful engagement, and communication to residents following a person-centered individual service plan. You will treat residents with respect and dignity, recognize individual needs, and encourage independence.

Requirements

  • High school diploma or general education degree (GED) preferred.
  • Three to six months related experience preferred and/or training in direct service with older adults living with dementia.
  • Knowledge of dementia, particularly of the Alzheimer's type.
  • May need to possess current state certification and follow regulations to maintain current certification.

  • Certifications related to dementia care.

Responsibilities

  • Provides dementia care including adapted ADLs, meaningful engagement, and communication to residents following a person-centered individual service plan.
  • Treats residents with respect and dignity, recognizes individual needs, and encourages independence.
  • Fosters a purposeful and engaging culture throughout the community.
  • Assists residents and provides input regarding the care plan and daily living activities.
  • Partners with residents to encourage independence in their care.
  • Participates in team behavioral expression meetings or discussions with community leadership.
  • Serves meals to residents in the dining room and may assist in preparing meals.
  • Encourages residents to eat a proper diet and take adequate fluids while respecting their right to free choice.
  • Records and reports changes in residents' eating habits to the supervisor.
  • Initiates and/or leads at least one Daily Path engagement program for residents.
  • Maintains residents' records daily in accordance with company policy and procedures.
  • Observes and reports the health and emotional condition of each resident.
  • Maintains a clean, safe, and orderly environment for the residents.
  • Performs general housekeeping duties.
  • Responds promptly to resident requests for assistance, including emergency situations.
  • Encourages teamwork through cooperative interactions with co-workers.
