About The Position

The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y. Jordan Health is now seeking a CHW/Patient Connection Specialist who, under the guidance of the PPC Program Manager, is primarily responsible for maintaining continuity and retention in care through assessments for HIV and Hepatitis C+ patients as required. Requirements

  • B.A. or B.S. in Psychology, Social Work, Counseling, Substance abuse counseling, Sociology, community health, or Public Health; or at least two years of experience in the field of HIV/AIDS.
  • One year of experience providing health education or case management, and familiar with psychiatric disorders, substance abuse, and cognitive impairments.
  • Fluent in English and Spanish
  • Valid NYS driver’s license.
  • Experience in health treatment: treatment plan, program implementation, and or direct delivery of health services.
  • Ability to deliver health education services and coaching.
  • Effective communication and documentation skills.
  • This position requires some expertise in Medicaid, Medicare, and ADAP insurance programs.
  • Sensitive to confidential information and HIPPA regulations.
  • BLS through the American Heart Association
  • NYS Infection Control Certificate
  • Strong writing and computer skills
  • Possess and utilize effective verbal and written communication
  • Ability to be self-motivated, work independently, and work as a team member
  • Ability to represent the agency in a professional manner within the community
  • Skills and competence to establish supportive, trusting relationships with people living with HIV/AIDS and respect doe client rights and personal preferences are essential.

  • Contribute to the efficiency and quality focus of Jordan Health teams.
  • Provide the highest quality care most efficiently when the clinician has all the resources necessary.
