About The Position

The Chronic Care Team Manager is a clinical professional who uses evidence-based practice & a patient-centric approach to work with the CCM team, their assigned patients on management of the patient’s chronic care conditions. The CCM Team Manager works collaboratively with the CCM team, Primary Care Physicians (PCPs), health care team & the patient’s family to coordinate patients care while decreasing inappropriate Emergency Department visits and readmissions to hospital. The CCM Team Manager will assists the other CCMs with issues that need extra support related to: patient care transitions including medication reconciliation & ensuring the patient has the appropriate DME. They also assists their patients and the other CCM team members patients to self- manage and maintain optimal level of health. Works with patients and their family to increase their knowledge level on health care conditions and provides information to improve their health. Assists the clinics and rest of Ambulatory Quality team with closing patient care gaps, updating health maintenance topics in Epic, assist with resolution of suspect conditions in value-based contracts, and ensuring medication adherence for payor quality incentive programs. This team also works with Transition Care Clinic & Care Suite with referrals as needed.

Requirements

• Graduate of approved School of Nursing with a current North Carolina RN license or RN multi-state licensure privilege through the Nurse Licensure Compact
• BSN required, MSN or MHA optional
• Minimum of two (2) years of clinical experience related to care coordination, case management
• Reliable transportation and valid driver’s license
• Minimum of three (3) years’ experience in supervisory responsibilities
