About this role The Chronic Care Nurse (RN) is responsible for managing and providing timely care to patients with chronic medical conditions by working closely with on-site physicians and mid-level providers. This position ensures that chronically ill patients are seen on a routine and scheduled basis according to their disease process. The Chronic Care Nurse (RN) also tracks and manages the overall care of these patients.

Requirements

  • Must be a graduate of an accredited school of nursing
  • Three (3) years of relevant clinical experience is required.
  • Currently licensed as a Registered Nurse in the state within which services are provided and possess and maintain current CPR certification.

Nice To Haves

  • Prior medical/surgical or emergency/trauma experience is preferred.
  • Previous corrections experience preferred.

Responsibilities

  • Maintain a comprehensive patient log and schedule for chronic care patients.
  • Assess patients as directed by physicians or mid-level providers and prepare patients for visits, ensuring all required diagnostic testing is complete.
  • Provide patient education, address special needs, schedule off-site consultations, and ensure off-site recommendations are presented to providers.
  • Maintain chronic care statistical data, review aftercare recommendations, conduct medication compliance studies, and maximize use of facility services.
  • Ensure compliance with National Commission on Correctional Health Care standards for chronic care programs.
