About The Position

The Chronic Care Management (CCM) and Quality Lead is a pivotal role responsible for overseeing and enhancing the Chronic Care Management and Quality programs at Sage Primary and Palliative Care. The successful candidate will have expertise in EMR systems, particularly Athena, a comprehensive understanding of CCM and quality billing, and a track record of leading teams or initiatives through practice changes. This role reports directly to the Program Manager and Director, with direct ownership of the Quality and CCM program.

Requirements

  • Bachelor’s degree in healthcare administration, nursing, or a related field
  • Minimum of 3-5 years of experience in a similar role within a healthcare setting.
  • Proven experience with Athena or other Electronic Medical Record (EMR) systems.
  • Strong understanding of Chronic Care Management and Quality billing.
  • Demonstrated ability to lead teams or programs through practice changes effectively.
  • Experience in providing educational support and training to clinical teams.
  • Excellent analytical, organizational, and communication skills.
  • Ability to work independently and manage multiple priorities in a fast-paced environment.
  • Familiarity with CMS billing standards and regulatory requirements.
  • Adhere to the highest standards of professionalism and patient confidentiality.
  • Maintain an up-to-date understanding of CCM and Quality best practices and CMS requirements.
  • Proactively identify opportunities for program development and enhancement.

Responsibilities

  • Drive the development and implementation of CCM and Quality improvement initiatives.
  • Lead change management efforts to achieve high-quality standards and practice transformation.
  • Ensure adherence to CMS billing standards and conduct internal audits to maintain compliance.
  • Oversee daily, weekly, and monthly reporting activities related to CCM and Quality metrics.
  • Perform data analysis and generate reports for program evaluation and improvement purposes.
  • Facilitate regular audits to ensure program alignment with CMS requirements and organizational goals.
  • Educate and support clinical and administrative staff on CCM and Quality initiatives.
  • Develop and lead training sessions to enhance team understanding and execution of CMS compliant practices.
  • Encourage a culture of continuous improvement and learning within the team.
  • Maintain open communication lines with the Program Manager and Director regarding program performance and initiatives.
  • Collaborate with cross-functional teams to align CCM and Quality strategies with overall organizational objectives.
