As a Chronic Care Coordinator, you’ll play a key role in supporting geriatric patients by ensuring continuity of care, managing chronic conditions, and helping patients navigate the healthcare system.

Requirements

• CNA, HHA, Medical Assistant certification, or higher
• Minimum 1 year of experience in home health, SNF, or primary care
• Knowledge of medical terminology and chronic disease processes
• Strong computer and EMR skills
• Excellent communication, organization, and follow-up abilities

Nice To Haves

• Chronic Care Management (CCM) experience preferred

Responsibilities

• Provide chronic care management for patients through phone calls and coordination
• Develop and maintain individualized care plans
• Ensure preventive care services are scheduled, tracked, and completed
• Conduct outreach to patients enrolled in Chronic Care Management (CCM) programs
• Coordinate care transitions, referrals, and communication with providers
• Support continuity of care across primary, specialty, and tertiary services
• Maintain accurate documentation using HIPAA-compliant EMR systems
