About The Position

Prestige ADHC Inc is seeking a Social Worker or Social Worker Assistant to provide essential medical and therapeutic services to medically compromised seniors, enabling them to remain active in their homes and communities. The role involves conducting assessments, case coordination, and providing counseling to improve the quality of life for participants, particularly those who are Chinese speaking.

Requirements

• Master's degree in Social Work from an accredited school of social work or a Bachelor's degree in Social Work with 1 year of relevant experience.
• Fluency in Mandarin or Cantonese is required.

Nice To Haves

• Experience working with seniors or in a community-based setting.

Responsibilities

• Complete social service initial assessments and reassessments for participants.
• Coordinate cases with outside services.
• Provide functional assistance for participants' personal care and program needs.
• Offer counseling for participants and families as needed.
• Actively participate in the CBAS Multi-Disciplinary Team.
• Collaborate with other health care personnel in participant evaluation and treatment.
• Prepare participant histories, individual plans of care, and necessary documentation.
