About The Position

The Children's Case Manager at Terros Health is responsible for providing effective case management and service coordination for families, particularly those involved with the Department of Child Safety. This role requires a compassionate and professional individual who can work flexibly with children and families during vulnerable times. The position involves treatment coordination, assessments, and direct service, including in-office and in-home support. The Children's Case Manager will work as part of an interdisciplinary clinical team, ensuring that families receive comprehensive care in accordance with the Arizona Practice Model.

Requirements

• Bilingual in Spanish and English preferred.
• High School Diploma, GED, or Associate's Degree in Behavioral Health Field.
• Bachelor's degree in Behavioral Health or related field with 1 year of experience working with children, or a Master's degree in Behavioral Health.
• Minimum of 3 years of behavioral health experience working with children if holding a GED/High School diploma.
• Valid Arizona Driver License and a minimum of 4 years driving experience.
• Must have or obtain a valid Arizona Fingerprint Clearance card (Level 1).
• Must pass a TB Test.

Responsibilities

• Assist with treatment coordination, including 6-month and annual assessments, and hospital admissions and discharges.
• Serve children involved with the Department of Child Safety who are not receiving other behavioral health services.
• Complete intakes as required by site needs and assist teammates with case coordination for additional referrals.
• Communicate and document client progress toward their family vision and treatment goals.
• Provide in-office and in-home family support and case management services.
