Duties/Responsibilities: The following duties are normal for this position. These are not to be construed as exclusive or all -inclusive. Other duties may be required and assigned. This position will: Assists families in the process of applying for Children’s Long Term Support Services as appropriate. Performs intake assessments and interviews collateral sources to determine if individual is eligible for services. Provide families with information, support, and opportunities as needed to enable them to make informed decisions. Authorizes and coordinates services to assist families to care for their children in their home. Provides and coordinates individualized comprehensive services to assist families in caring for their children in their homes. Understand the Service Delivery System, value natural supports, acknowledge the limitations of budgets, funding, costs, and choices; ensuring resources are effective, efficient, and have value to the participant. Coordinate and facilitate the development of Individual Service Plans that meet the needs of families and maintain measures of progress in meeting outcomes. This includes on-going monitoring and targeted case management practice. Creates a budget and manages funding allocated to each case. Reviews invoices and service codes and authorizes invoices for payment. Maximize and support the family’s ability to be self-sufficient while accessing community resources relevant to their needs. Provide Support and Service Coordination Services efficiently and effectively which reflects the philosophy of self-determination of the Children’s Long-Term Support Program and Children’s Community Options Program. Document all support and service coordination activities and complete paperwork as required. Work to build and maintain positive community partnerships and participate in meetings and training opportunities to enhance skills. Be available and responsive to families and team members by maintaining flexibility, confidentiality and punctuality. This may include working unstructured hours, including evening hours. Principal Accountabilities: Ensures each participants legal rights, safeguards, and confidentiality are upheld. Ensures compliance with Federal, State, County, and department rules, regulations, policies, and procedures. Provides accurate and complete assessment/applications, progress notes, and other written information as required in accordance with State, Federal, and department standards and schedules. To meet on-going training requirements of funding sources and to maintain knowledge necessary to assess, plan, and monitor supports and to keep current in rules, regulations, and community resources. Responds to families, service providers, and co-workers in a timely and courteous manner. Ensures the implementation of department values, goals, and procedures. Demonstrates professional conduct. To mobilize appropriate resources that are effective, efficient, and have value to those that use funding or services.
