South Central Mental Health is currently recruiting Children's Case Managers. Overall endeavor is to prevent hospitalization or out-of-home placement and to reintegrate consumers into the community. Specialist is an active participant in meeting related administration or treatment planning processes. Judgment: Sound judgment is used in ongoing assignments. Maintains a positive, professional and supportive relationship with consumers. Maintains a positive and professional relationship with staff from SCMH and other agencies. Confidentiality is maintained at all times. Specialists keep other staff informed of unusual situations regarding consumers. Documentation: Progress notes are accurate and reflect progress toward treatment goals. Documentation is current and turned in weekly.
