About The Position

As an HHSC Care Manager, you'll serve as the primary point of contact for children with chronic conditions or behavioral health challenges and their families. You’ll coordinate care across providers, schools, and support systems to ensure a comprehensive, child-centered plan of care is in place and followed. Your work will span Brooklyn and Queens, meeting children and families where they are—at home, in the community, or virtually.

Requirements

• Bachelor’s degree in social work, psychology, sociology, education, nursing, public health, or a related field
• At least 2 years of relevant experience working with children/families
• Knowledge of Medicaid, Public Assistance, SNAP, and other community-based services a plus
• Valid NYS Driver’s License or reliable transportation for community visits

Responsibilities

• Conduct comprehensive assessments and develop individualized care plans
• Coordinate medical, behavioral health, educational, and social services
• Serve as a liaison between families, providers, schools, and support systems
• Track and document progress toward health, social needs, and wellness goals
• Coordinate transitional care from Emergency Room/Inpatient Admissions to ensure safe discharge to the community
• Advocate for children’s needs while empowering families
• Attend regular supervisory and case conference sessions, trainings and other meetings, as assigned.
