About The Position
Morrison Center is seeking a Children’s Case Manager , to join our dynamic, multidisciplinary team in Scarborough, Maine Case Managers develop individualized support plans for individuals served, using a person-centered planning process; analyze support strategies and assure that services received by clients adequately meet their needs; and assist individuals and their families to identify strategies that will enable them to meet their goals.
Requirements
- Bachelor’s degree
- Ability to work in person
Nice To Haves
- One preferred year experience in Children's Services or two years in social services
- Experience working with individuals with intellectual/developmental disabilities, preferred
Responsibilities
- Working with children who have intellectual and/or developmental disability
- Managing a case load of 25 - 30 clients
- Working with children as young as 6 months – 20 years old.
- Documentation in accordance with regulatory agencies
- Case Manager must represent Morrison Center in the community and act as a liaison with other agencies.
- Perform monthly home visits, school observation
- Support families at IEP meetings, team meetings and medical appointments
- Complete 90 day continued stay reviews with the family
