Our multidisciplinary team serves youth ages 3–21 who are experiencing mental illness or serious emotional challenges. We are seeking a Children’s Assertive Community Treatment (ACT) Community Case Manager to join our team and support children and families in achieving stability, growth, and meaningful progress toward wellness. This is a full-time, 40 hour per week position. The ACT model is an evidence-based practice that provides intensive, highly integrated, community-based mental health services. Deliver counseling and Functional Support Services (FSS) in home, school, and community settings. Provide Therapeutic Behavioral Services (TBSI) to teach and reinforce coping strategies. Offer Family Training and Counseling (FTC) to strengthen support systems and promote resilience. Provide Crisis Intervention (CI) when children experience acute exacerbation of symptoms. Collaborate closely with ACT team members, schools, medical providers, and natural supports to ensure wrap-around care. Maintain timely and accurate documentation for all services provided. Meet required productivity expectations (16 case management contacts per month, 18 hours of FSS weekly; eligible for incentive pay when exceeding KPI expectations at a rate of $40/hour). Participate in committees as assigned and maintain adherence to confidentiality, professional standards, and agency policies. Uphold all licensing, accreditation, and certification requirements.
