About The Position

The Child & Youth Care Manager is responsible for coordinating comprehensive, family-driven, and youth-guided care for children and adolescents enrolled in services with AthenaPsych. This role supports youth with complex medical, behavioral health, developmental, and social needs by developing individualized plans of care, coordinating services, and advocating for access to community resources. The Care Manager collaborates closely with families, providers, schools, and community agencies to ensure integrated care across medical, behavioral health, and social systems. The position emphasizes strengths-based, trauma-informed, and culturally responsive care planning to support youth in achieving optimal health, stability, and functioning in the community.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, or related field required.
  • Minimum 2 years of experience working with children, adolescents, or families in behavioral health, social services, care management, or related field.
  • Strong communication, organization, and problem-solving skills.
  • Ability to work independently and collaboratively with multidisciplinary teams.
  • Experience in care coordination or case management programs.
  • Knowledge of child welfare, developmental disabilities, or behavioral health systems.

  • Bilingual in Spanish preferred.

Responsibilities

  • Conduct comprehensive assessments to identify strengths, needs, and service gaps.
  • Develop, implement, and monitor individualized, family-driven Plans of Care.
  • Coordinate medical, behavioral health, educational, and social services.
  • Facilitate care team meetings and interdisciplinary collaboration.
  • Support transitions between levels of care or service systems.
  • Provide outreach and ongoing engagement with youth and caregivers.
  • Educate families on available services, benefits, and treatment options.
  • Advocate for access to medical, behavioral health, social, educational, and supportive services.
  • Maintain accurate, timely documentation of services, assessments, and care plans.
  • Conduct home visits, school visits, and community-based meetings as required.
  • Collaborate with external providers, social service agencies, and care team members.
