The Licensed Psychologist is a vital member of a multi-disciplinary team composed of psychiatrists, APPs, psychologists, therapists, nursing, and other disciplines to provide holistic or comprehensive medical and mental health services to patients. They interview patients and give diagnostic tests as clinically indicated. The Psychologist provides individual, family or group psychotherapy, and collaborates with the behavioral health team to develop and implement evidenced-based treatment and interventions. Mental Health Integration (MHI) is collaborative mental health care that is integrated into everyday primary care practice. PCPs and office staff collaborate with care managers and mental health specialists to implement individualized strategies for patients and families. As the Mental Health Specialist within the MHI model, you will work directly with the Primary Care Provider and team to clarify complex patient diagnoses, plan effective treatment, and provide solution-focused interventions. This improves clinical decisions, helps patients and families receive services within primary care, and reduces PCP burden. Essential Functions Assesses the patient's medical history, mental health history, and current presentation to understand the patient's needs from treatment. Diagnoses and treats various mental, emotional and behavioral disorders, which may include psychotherapy or psychological testing. Develops a plan of care either individually or with input from the patient, patient family/significant others, physicians and other members of the healthcare team. Provides treatment based on patient need and evidenced-based interventions. Refers patients to other services to improve their well-being. These referrals may include community services, treatment programs, support groups, and/or psychiatrists (or other mental health providers). Properly documents pertinent information regarding the patient's plan of care. Consults with the multi-disciplinary team to develop treatment plans, behavior plans and discharge plans.
