We are seeking a talented Child and Family Team Specialist to join our team of nearly 500 employees. The Youth and Family Partnership (YFP) is a Program which serves Monroe County youth and their families, who are at risk of out of home placement, have emerging and/or unmet behavioral health challenges and demonstrate complex needs that require the involvement of more than one system. The primary goal of the YFP is to provide strength-based wrap-around services to youth and their families, utilizing an integrative care model, to ensure that customizable services and supports are provided to meet identified needs, within the least restrictive, most normative environment that is clinically appropriate. Referrals are only accepted from MCDHS Child Protectives Services Investigation Teams, SPOA and FACT. CCSI is partnering with Monroe County Department of Human Services (MC DHS) to hire a Child and Family Team Specialist. This position is employed by CCSI and working on behalf of Monroe County Department of Human Services (MC DHS). In this role, you will Coordinate comprehensive planning process that utilizes a family’s strengths, as well as formal and informal supports, to develop a Plan of Care using strategies outside the parameters of standard service delivery to meet a youth’s and their family’s needs

Requirements

  • Bachelor of Social Work/related field plus 4 years’ experience, OR Master of Social Work/related field plus 2 years’ experience.
  • Must obtain and maintain certification to administer the Child and Adolescent Needs and Strengths (CANS-NY) tool.
  • Valid Class D New York State Driver License and access to motor vehicle to meet transportation requirements of position.
  • Satisfactory completion of required background screenings: criminal, probation, drug, Statewide Central Register.
  • Ability to work flexible hours to meet the needs of the families.
  • Respect the confidential nature of all CCSI and CCSI customer information you are exposed to in the course of your work performance. Staff working directly with protected health information /electronically stored protected health information, personally identifiable information, and other patient/consumer/customer data, will abide by the specific procedures and policies outlined by CCSI’s Code of Conduct and CCSI’s Privacy and Security Policies, New York State regulations and Federal regulations.

Responsibilities

  • Conduct family assessments to identify strengths, needs and cultural attributes; assessments are conducted in a respectful way, while honoring each family’s uniqueness, resilience, hopes and dreams.
  • Build Child and Family Teams utilizing natural supports, community supports, providers and other identified resources.
  • Educate CFT team members on System of Care values.
  • Regularly facilitate monthly CFT meetings, developing an individualized Plan of Care based on family’s unique strengths and needs, which maximizes natural supports, using services only when necessary.
  • With team, implement and monitor Plan of Care with goal of maintaining the child safely at home or in the community.
  • Monitor and measure effectiveness of Plan and adapt Plan as necessary.
  • Partner effectively and collaboratively with Monroe County Departments of Child and Family Services and Office of Mental Health, as well as education system, medical and clinical providers, community resources and informal supports.
  • Embrace, demonstrate and model Monroe County System of Care values, including family driven, youth guided, trauma informed, community-based, best practice oriented, and culturally and linguistically competent.
  • Ensure family and youth participation in all aspects of planning, service delivery and evaluation.
  • Ensure smooth transitions for youth and their families between systems and supports, through consistent communication, planning and follow up among involved systems, agencies, providers and the youth and family.
  • Stay supportive and respectful of team members and works to resolve all conflicts in professional manner.
  • Complete all systems requirements of MCDHS and all other required documentation in thorough and timely fashion.
  • Adhere to program expectations regarding best practice, punctuality, attendance, and use of work time.
  • Work to enhance performance through active participation in team meetings, training opportunities and supervision.
  • Adhere to program expectations regarding importance of professional boundaries.
  • Fulfill other activities as assigned by Youth and Family Partnership Supervisor.
  • Perform other related duties as assigned.
