About The Position

Case Managers will provide quality case management services to youth with a severe emotional disturbance (SED). The majority of the Case Manager's time will be spent providing services to youth in the family home, educational setting, program site or other community settings.

Requirements

  • Bachelor's Degree or be equivalently qualified through a combination of related work experience and education, with one year of experience substituting for one year of education.
  • Meet specifications outlined by KDADS licensing policy.
  • Completion of background checks, drug screening process, and motor vehicle record check.
  • Familiarity with mental health theory and practice and ability to relate a positive approach to complex mental health issues.
  • Effective interpersonal skills and the ability to communicate both orally and in writing.
  • Computer proficiency to use electronic health record and other information systems.
  • Ability to establish therapeutic relationships with clients and effective working relationships and community agencies.

Responsibilities

  • Provide quality, strengths based case management services to SED children and families.
  • See CPST and TCM services descriptions below.
  • Ability to work professionally and in conjunction with other mental health center staff.
  • To assure that referrals for case management services are acted upon timely, typically within 3 business days of referral.
  • To assure that clients will be assessed via the strengths/needs assessment tool.
  • To assure timely completion, documentation, and/or submission of service notes, assessments, and data reporting within agency required timeline.
  • Meet direct service hour expectation of 95 hours per month.
  • Ability to work a flexible schedule, as needed, to meet client needs and meet necessary work responsibilities.
  • To complete training requirements as necessary for provision case management and additional Rehabilitation Services, as determined by supervisor and KDADS.
  • To provide crisis intervention according to SRMHW's policy.
  • Other duties as assigned.
