About The Position

The Charge Nurse at Methodist Transitional Care, plays a critical role in delivering high-quality nursing care to patients in a transitional care setting. This position involves providing direct patient care, monitoring patient health status, and assisting with the implementation of individualized care plans under the supervision of registered nurses and physicians. The LVN/RN will be responsible for ensuring patient comfort, safety, and well-being while supporting the interdisciplinary healthcare team to facilitate smooth patient transitions from hospital to home or other care facilities. This role requires a compassionate and detail-oriented professional who can effectively communicate with patients, families, and healthcare team members to promote optimal health outcomes. Ultimately, the LVN/RN contributes to the overall mission of Methodist Transitional Care by enhancing patient recovery and quality of life through skilled nursing care and patient education.

Requirements

  • Current and valid LVN/RN license in Texas.
  • Completion of an accredited vocational nursing program.
  • Basic Life Support (BLS) certification.
  • Minimum of one year of clinical nursing experience, preferably in a transitional care or similar healthcare setting.
  • Ability to pass background checks and meet health screening requirements.

Nice To Haves

  • Experience working in transitional care, rehabilitation, or long-term care facilities.
  • Advanced certifications such as IV therapy or wound care specialty.
  • Familiarity with electronic health record (EHR) systems used in healthcare settings.
  • Strong communication and patient education skills.
  • Demonstrated ability to work effectively within interdisciplinary healthcare teams.

Responsibilities

  • Provide direct nursing care to patients, including administering medications, wound care, and monitoring vital signs.
  • Assist in the development and implementation of individualized patient care plans in collaboration with registered nurses and physicians.
  • Observe and document patient conditions and report any changes or concerns promptly to the healthcare team.
  • Support patients with activities of daily living, ensuring their comfort and safety throughout their stay.
  • Educate patients and their families on health management, medication administration, and post-discharge care instructions.
  • Maintain accurate and timely patient records in compliance with healthcare regulations and facility policies.
  • Participate in interdisciplinary team meetings to coordinate patient care and discharge planning.
  • Adhere to infection control protocols and safety standards to maintain a clean and safe care environment.
