About The Position

The Courville at Nashu a is a Senior Healthcare Community in Nashua, NH. We are locally owned and operated and committed to providing personalized service and care. We’re looking for a Charge Nurse (RN/LPN) to join our Short-Term Rehab unit. The successful candidate will be an experienced nurse, RN preferred, who is compassionate, energetic, and dedicated to the highest standards of care. Job Description : The Charge Nurse is primarily responsible for overseeing day-to-day patient care, supervising, and directing nursing staff. The Charge Nurse reports directly to the Unit Manger and the Director of Nursing to ensure quality patient care within the skilled nursing unit. The Charge Nurse is required to be dependable and personable to communicate with residents and their families. The Charge Nurse ensures the Skilled/Rehab unit complies with federal and state regulatory standards and follows company policies and procedures. This position is FT, 40-hrs/week, M-F, with a rotating on call (weekends/holidays)

Requirements

  • Must have a NH Nursing License RN preferred; will consider LPN
  • Must have experience working in a short-term rehab unit
  • Must have Charge Nurse or Unit Manager Experience
  • Strong assessment, prioritization, and clinical judgement skills
  • Must be able to lead in a high resident turnover, fast paced environment
  • Excellent communication and teamwork skills.

Responsibilities

  • Maintaining and restoring health of resident patients
  • Acting as a point of contact between residents and their families
  • Addressing and preventing concerns from family members
  • Assists in developing, implementing and adjusting patient care plans as needed
  • Supervises, counsels, and provides discipline, when necessary, to nurse staff to ensure the highest quality of care possible
  • Ensures their unit meets federal and state regulatory standards, as well as company policies and procedures
  • Collaborate with the Unit Manager, Director of Nursing, Nurse Practitioners and Physicians to plan and maintain resident care
  • Completes admission and discharge assessments
  • Oversees and audits changes made to care plans
  • Delivers hands-on care and medicine distribution as needed
  • Audits and completes charting, evaluations, medical forms, and other administrative tasks as directed
  • Identifies procedural problems and works with the Unit Manager and Director of Nursing to resolve issues
  • Evaluates, prioritizes and meets the care needs of each resident
  • Attends and participates in clinical review meetings, PPS meetings, at risk meetings, monthly QA/PI meetings, care plan meetings, discharge meetings as directed.
  • Monitor / review documentation on the unit provided by the LNA and nursing staff
  • Participate in committees as requested by Administrator / Director of Nursing Services.
  • Ensure work assignments are made for LNA/Nursing staff, including breaks/meal breaks and staff is compliant with assignments
  • Ensure physician orders are being processed and followed
  • Participate in Annual Survey/Complaint surveys or any other survey process as directed by the Administrator and / or the Director of Nursing Services.
