About The Position

The Chaplain plays a vital role in providing spiritual care, emotional support, and guidance to individuals facing challenging life circumstances, including patients, families, and staff within healthcare or community settings. This position requires the ability to offer compassionate grief counseling and pastoral care tailored to diverse beliefs and backgrounds, ensuring respect for patient rights and dignity. The Chaplain will develop and implement programs that foster spiritual well-being and volunteer engagement, contributing to a holistic approach to care. By exercising independent decision-making, the Chaplain addresses complex emotional and ethical issues sensitively and effectively. Ultimately, this role supports the overall mission of the organization by promoting healing, hope, and resilience through dedicated stewardship and pastoral presence. As a Hopestone Hospice Chaplain, you will:

Requirements

  • One year of experience as a Chaplain in health care preferred
  • One or more years of Hospice experience required
  • CPR Certification
  • Valid Driver’s License
  • Valid Auto Insurance
  • HomeCare HomeBase experience preferred

Responsibilities

  • Assess hospice patients and support systems to identify spiritual needs as evidenced by POC, documentation, clinical records, and IDG conference minutes.
  • Provide support for the spiritual needs of the patient/family.
  • Attend interdisciplinary team meetings and participate in the patient care planning process.
  • Use effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
  • Meet mandatory continuing education requirements of the agency.
  • Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements.
  • Promote Agency philosophy and administrative policies to ensure quality of care.
  • Demonstrate understanding of coordination and continuity of care by collaborating with the IDG.
  • Accept clinical assignments that are consistent with education and competence to meet the needs of the patients/families.
