About The Position

The position involves providing spiritual support to patients and families in a hospice setting, ensuring care aligns with their belief systems. The role includes conducting spiritual assessments, managing bereavement follow-ups, and collaborating with healthcare teams to enhance sensitivity to spiritual concerns during terminal illness and loss.

Requirements

  • Bachelor's Degree
  • Completion of Two (2) Units of Clinical Pastoral Education
  • Six (6) Months Experience

Nice To Haves

  • Masters of Divinity Graduate or Doctoral Degree in Biblical Studies
  • Theological or Spiritual Studies
  • Graduate or Doctorate in Pastoral Counseling
  • Ordination from a Recognized Religious Body into a Ministry-related Field

Responsibilities

  • Provide spiritual support to patients and families according to their belief systems.
  • Complete initial spiritual assessment within 24 hours of contact for all assigned patients.
  • Contact all patients/families within one week of the patient's admission to Hospice services.
  • Document assessments, interventions, and planning in accordance with organizational timelines, updating care plans every two weeks or with changes in patient status.
  • Perform and arrange funeral services as requested by patients/families and assist in organizing memorial services for staff and families annually.
  • Listen to customers (patients/families) to understand their concerns and needs.
  • Recognize grievance situations and manage complaints according to organizational procedures.
  • Provide spiritual care and support to patients/families regardless of race, gender, religious background, or lifestyle, and offer additional resources as needed.
  • Work with staff, clergy, and community groups to enhance sensitivity to spiritual concerns of patients/families experiencing terminal illness and loss.
  • Participate in weekly team meetings and provide input on current patients, as well as conduct one education session per year for Hospice staff.
  • Demonstrate flexibility with job responsibilities and complete Hospice Consents as required.
  • Adhere to HMH's Organizational competencies and standards of behavior.
  • Lift a minimum of 10 lbs., push and pull a minimum of 10 lbs., and stand for a minimum of 4 hours a day.
