In addition to the responsibilities listed below, this role is also responsible for fostering relationships with patients, their families and loved ones, and staff related to a patients terminal illness in a home or community; guiding the completion of patient documentation and fulfillment of physician orders (e.g., tracking home visit frequencies) with respect to hospice regulatory compliance; driving the provision of both individual and group-level spiritual counseling services focusing on end-of-life in home and community settings, ensuring complex cases are solved; and providing non-RN pain assessments and recommending appropriate interventions (e.g., breathing techniques, guided meditation) to comfort patients, their families and loved ones, and staff.

  • Provides during and after end-of-life support and counseling to patients, loved ones, and staff.
  • Documents spiritual assessments and interventions in the patient care record.
  • Follows established charting methodologies in accordance with policies and procedures.
  • Role models cultural sensitivity when interacting with patients, their families and loved ones, and team members.
  • Identifies and/or tends to spiritual or religious needs including providing supportive counseling.
  • Provides spiritual resources to patients, loved ones, and staff.
  • Evaluates the quality and satisfaction of spiritual care services.
  • Masters degree in Divinity, Theology or related field from an accredited university.
  • Minimum two (2) years of pastoral care experience in a Hospital, Clinic, Ambulatory, Home Health/Palliative Care, or Hospice setting.
