We are excited to continue to grow our mission family! Our Mission Hospice team is looking for a Chaplain! End of life transitions are crucial to the care we provide to patients and their families. We are looking for a kind, compassionate, and spiritual, guide to provide care and comfort to our patients and families. We are looking for Per Diem support. Visit Rate: $40 NVA Rate: $25-$28 SOC Rate: $40 Coverage Area Includes: Salt Lake and Utah County.

Requirements

  • Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree).
  • Experience working with death and dying, individuals/family/caregiver
  • Ability to work as member of interdisciplinary group.
  • Comfort in an interfaith setting.
  • Ability to accept different lifestyles, cultures, beliefs, and values.
  • Ability to network with community clergy and congregations.
  • Knowledge of and commitment to hospice philosophy of care.
  • Hospice experience preferred.

Responsibilities

  • Plan, coordinate, and implement spiritual support activities that reflect the beliefs and values of the patient and family.
  • Perform a spiritual assessment within five days of admission to hospice.
  • Attend interdisciplinary team meetings and collaborate with the team in developing the plan of care and ensure it reflect spiritual needs.
  • Coordinate with or refer to local clergy as requested by the patient and family.
  • Documents clearly, accurately and timely in the patient record.
  • Follow agency policy & procedures and adheres to professional boundaries.
  • Complies with HIPPA and confidentiality agreement.
  • Assist families in planning funeral and memorial services when indicated.
  • Collaborate with the bereavement coordinate to plan hospice memorial services for families of patients that have died.
  • Attends interdisciplinary team meetings and is proactive in reporting current issues and collaborating with the team in developing interventions for the plan of care.
  • Coordinates care with other professionals to meet the emotional and psychological needs of the patient and family.
  • Conducts a risk assessment of complicating bereavement factors.
