About The Position

The Hospice Chaplain at Allay Home and Hospice plays a crucial role in providing spiritual care services to patients and their families, ensuring an optimal care experience. This position involves working closely with an interdisciplinary team to assess and meet the spiritual needs of hospice patients, while also serving as a liaison to community chaplains and spiritual counselors.

Requirements

• Graduate of an accredited theology or chaplaincy program (BD, MDiv or equivalent theological degree)
• Experience working with death and dying, hospice experience preferred
• Comfort in an interfaith setting
• Ability to accept different lifestyles, cultures, beliefs and values
• Knowledge of and commitment to hospice philosophy of care

Responsibilities

• Assure spiritual assessment of patients and families/caregivers in the hospice program and provide direct spiritual care as appropriate
• Participate in the development of an individualized plan of care to meet spiritual needs
• Serve as a liaison to community chaplains and spiritual counselors
• Maintain records and documentation of services provided
• Attend patient care conference and interdisciplinary group meetings
• Provide funeral or memorial services for patients as requested
• Ensure compliance with all state and federal legal and regulatory requirements
• Additional responsibilities as assigned
